Baseline Medical Care Model

Baseline Medical is a mobile medical care platform designed to coordinate non-emergency medical services delivered where life happens, including homes, hotels, workplaces, and other appropriate locations.

Baseline Medical is a consumer-facing brand. Medical services are provided by licensed clinicians affiliated with Baseline Medical Group, PC, or other affiliated professional entities, in accordance with applicable state laws and clinical regulations.

Care may include Registered Nurse visits, Nurse Practitioner oversight, clinical appropriateness review, mobile evaluation and treatment support, telemedicine-enabled coordination, and escalation guidance when higher-acuity care is appropriate.

All care decisions are made by licensed clinicians exercising independent professional judgment.

No Guarantee Of Care, Treatment, Or Availability

Submitting a care request does not guarantee acceptance as a patient, treatment eligibility, provider availability, scheduling availability, service coverage eligibility, prescription issuance, clinical approval, visit confirmation, or completion of care.

Baseline Medical reserves the right to decline, reschedule, redirect, or discontinue services when clinically appropriate, operationally necessary, or required by law.

Patient–Provider Relationship

A patient–provider relationship is not established solely through website access, educational content, marketing communications, care-request submission, intake completion, scheduling coordination, SMS or email communications, insurance or payment inquiries, or administrative interactions.

A patient–provider relationship is established only after a licensed clinician affiliated with Baseline Medical Group, PC or an affiliated professional entity accepts the patient for care, required clinical appropriateness review is completed, any legally required consent processes are satisfied, and care is initiated under applicable professional standards and state law.

Telemedicine, Coordination, And Hybrid Care Model

Certain components of Baseline Medical services may involve telemedicine interactions, remote clinical oversight, asynchronous intake review, digital triage workflows, messaging-based coordination, hybrid RN + NP care models, virtual follow-up interactions, remote symptom review, and operational scheduling coordination.

Telemedicine availability may vary based on state licensure requirements, clinical appropriateness, provider availability, patient location, technology limitations, and regulatory restrictions.

Remote communications, digital intake, asynchronous review, or operational messaging do not independently establish treatment eligibility, guarantee prescribing, or guarantee acceptance for care.

Clinical Oversight And Independent Judgment

Registered Nurses may perform in-person assessments, logistics coordination, symptom review, treatment administration, monitoring, and escalation support within the scope of their licensure and clinical protocols.
